Periodic Health Assessment
Your PHA is due every 12 months for all members (PRAP, AuoF, Fly, and Non-Fly components). Please check the MHS GENESIS Patient Portal for any messages about labs or screenings that may be required to finish your Individual Medical Readiness (IMR) requirements with your PHA. If you have any pending labs, you should receive a message that you can walk-in to the lab once orders are placed.
Step 1: Complete the Periodic Health Assessment Questionnaire (PHAQ)
- For Air Force and Space Force personnel, visit the My IMR site
- For Army personnel, visit MEDPROS portal
- Once complete, please send BOMC a message on the MHS GENESIS Patient Portal to Offutt BOMC (search "Offutt BOMC"). Without your notification we’re unable to track your PHA requirements and completion.
- For Navy, Marines, and Coast Guard visit the ePHA portal
- Once complete, please send BOMC a message on the MHS GENESIS Patient Portal to Offutt BOMC (search "Offutt BOMC"). Without your notification we’re unable to track your PHA requirements and completion.
Step 2: Schedule your PHA
- If you’re on Flight Status
- Schedule an in-person Fly PHA/MHA and audiogram by calling 402-232-2273, option 1,1.
- Fly PHAs are not available for online scheduling.
- You must have your Audiogram within 30 days before your Fly PHA appointment.
- If you’re on Arming Use of Force or PRP Status
- The PRAP Clinic will schedule your Occupational Health/MHA/PHA appointment.
- Please contact them at 402-294-9314.
- If you’re inactive Guard or Reserve, Coast Guard, deploying within the next 30 days or PCSing
- Schedule your MHA through our appointment line by calling 402-232-2273, option 1,1.
- If you’re not on Flight status, Arming Use of Force Status, and not deploying or PCSing within 30 days
- Call the VMC Virtual Appointment Management Office at 844-863-3236 to schedule a combined MHA and PHA virtual appointment.
- The VAMO is open Monday - Friday from 6 a.m. to 7 p.m. CST.
Deployment-Related Health Assessment
Deployment-Related Health Assessment 1
Your DRHA must be completed within 120 days of Estimated Departure Date. To begin, log in to the My IMR site and complete the Deployment-Related Health Assessment Questionnaire (DRHA or DD Form 2795). Once finished, you’ll be prompted to schedule your in-person DRHA1 appointment. Send a MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445.to schedule your DRHA.
If you're identified as having a waiver-able condition during your DRHA, you’ll be notified and processed through BOMC for a waiver. Waivers require medical stability. If you’re on a profile with a mobility restriction or AAC Code 31 you won’t be deployable at that time. The waiver process can take time; BOMC or Public Health will contact you once your waiver has been approved or denied.
Deployment-Related Health Assessment 2
DRHA2 must be completed within 30 days after returning from deployment, before going on leave or R&R. To begin, log in to the My IMR site and complete the Deployment-Related Health Assessment Questionnaire (DRHA or DD Form 2796). Once finished, you’ll be prompted to schedule your in-person DRHA2 appointment. Send a MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445 to schedule your DRHA.
Deployment-Related Health Assessment 3/4/5
DRHA3/4/5 are completed via phone call or telehealth. To begin, log in to the My IMR site and complete your Deployment-Related Health Assessment Questionnaire (DRHA or DD2796). Once finished, you’ll be prompted to schedule your DRHA3/4/5 virtual appointment. Send a MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445 to schedule your DRHA.
Medical Profiles – Fitness Restrictions
Active Duty Service Members (Air Force, Navy, Army, Marines)
Please schedule appointments with your primary care provider. Profiles are only created when indicated by the specific physical and/or mental conditions discussed in a provider appointment, and a provider will profile you appropriately per medical standards and regulations. If you’re having issues with obtaining required documentation (AF469 or sister service equivalent), please reach out to your PCM team utilizing the MHS GENESIS Patient Portal. Air Force personnel can retrieve their current AF469 profile by logging onto the My IMR site. If you have an active profile you will see a link for AF469 under the profile column from your IMR homepage.
For Air Force members that have Self-Certify profiles, please view instructions on how to certify your condition and follow up actions.
National Guard and Reserve Components
If you’re in the Guard or Reserve component and your primary military treatment facility is Offutt AFB, our BOMC assists with coordinating your profiles when you are seen off-base. If you live outside the Offutt AFB area and you have a civilian Health Care Provider or PCM that indicates you need to be put on a profile please send a message to Offutt BOMC (search "Offutt BOMC") via the MHS GENESIS Patient Portal. You must send us medical records from your off-base provider through a MHS GENESIS message or fax them to 402-294-7314 to generate a profile from a military health care provider. Failure to do so will result in a cancellation of your request, without supporting documentation BOMC will not start any notifications to have your records reviewed for profiling purposes.
