Periodic Health Assessment

Annual Requirements (Testing/Screenings)
Please check the MHS GENESIS Patient Portal for any messages about labs or screenings that may be required to finish your Individual Medical Readiness requirements with your PHA. If you have any pending labs, you should receive a message that you can visit the lab on a walk-in basis once orders are placed. 

Air Force

Your PHA is due every 12 months for all members (PRAP, AuoF, Fly, and Non-Fly components). To start the process please visit the My IMR site and complete your Periodic Health Assessment Questionnaire (PHAQ). 

Air Force Non-flyers & Arming Use of Force 
After completing the PHAQ, you’ll be prompted to complete your Mental Health Assessment. Schedule your MHA online through the MHS GENESIS Patient Portal appointments tab, search for “Military Medicine Telephone Visit” and search “Offutt” for Air Force Medicine Offutt Flight BOMC. The appointment line can also schedule MHA appointments, contact them at 402-232-2273, Option 1,1. MHA is by appointment only.  

Once you’ve completed your PHAQ and MHA, the Offutt Medical Group will complete a record review before signing off your PHA. Please be patient as this may take some time to fall off your IMR. 

 **If you’re Arming Use of Force, your PHA has an in-person requirement to coincide with your occupational health requirements. Your unit scheduler will contact the clinic to schedule/reschedule your appointment. You must complete your PHAQ before making an appointment.** 

Flyers (DD Form 2992 Holders) 
After completing the PHAQ, you’ll need to contact the Flight and Operational Medicine Clinic to schedule a Fly PHA/MHA. Flyers are required to be seen in person for their PHA and MHA in one appointment. You must have your audiogram within 30 days before your Fly PHA appointment. Please make an appointment for your Audiogram and Fly PHA by calling 402-232-2273, Option 1,1. Fly PHAs are not available for online scheduling. 

Sister Services

To start the process, fill out the respective sister service component questionnaire through the ePHA portal (Navy) or MEDPROS portal (Army). Once complete, please send BOMC a message on the MHS GENESIS Patient Portal to Offutt BOMC (search "Offutt BOMC"). 
Without your notification we’re unable to track your PHA requirements and completion. We apologize for any inconvenience this may cause; this allows BOMC to appropriately track and complete your PHA in a timely manner, so you don’t go overdue. 

Non-flyers & Arming Use of Force

Deployment-Related Health Assessment

Deployment-Related Health Assessment 1 
 
Your DRHA must be completed within 120 days of Estimated Departure Date. To start the process, please log onto the My IMR site and complete the Deployment-Related Health Assessment Questionnaire (DRHA or DD Form 2795). After completion, you’ll be prompted to complete your DRHA1 appointment. This appointment must be completed in person;  no virtual option is available since this is an AFI requirement. DRHA is by appointment only; check in at the Flight Medicine Clinic. Please send an MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445 to schedule your DRHA. 
  
If you’re identified at your DRHA as having a condition eligible for waiver, you’ll be notified and processed through BOMC. Waivers require medical stability, and if you’re on a profile with a mobility restriction or AAC Code 31 you won’t be deployable at that time. If you require a waiver, they take time. BOMC or Public Health will call you when the waiver process is complete to notify you of acceptance or denial. 

Deployment-Related Health Assessment 2 

DRHA2 must be completed no later than 30 days after returning from deployment. To start the process, log onto the My IMR site and complete the Deployment-Related Health Assessment Questionnaire (DRHA or DD Form 2796). After completion, patients will be prompted to complete the DRHA2 appointment. This appointment must be completed in person; no virtual option is accepted, as this is an AFI requirement. Patients must complete their DRHA2 before going on leave or R&R. DRHA2 is by appointment only; check in at the Flight Medicine Clinic. To schedule DRHA, send an MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445. 

Deployment-Related Health Assessment 3/4/5 

DRHA3/4/5 are completed via phone call or telehealth. To start the process please by log onto the My IMR site and complete your Deployment-Related Health Assessment Questionnaire (DRHA or DD2796). After completion you’ll be prompted to complete your DRHA3/4/5 appointment. DRHA3/4/5 are completed by appointment only but will be conducted via phone. Please send an MHS GENESIS Patient Portal message to Offutt BOMC (search "Offutt BOMC") or call 402-232-1445 to schedule your DRHA. 

Medical Profiles – Fitness Restrictions

Active Duty Service Members (Air Force, Navy, Army, Marines) 
Please schedule appointments with your primary care provider. Profiles are only created when indicated by the specific physical and/or mental conditions discussed in a provider appointment, and a provider will profile you appropriately per medical standards and regulations. If you’re having issues with obtaining required documentation (AF469 or sister service equivalent), please reach out to your PCM team utilizing the MHS GENESIS Patient Portal. Air Force personnel can retrieve their current AF469 profile by logging onto the My IMR site. If you have an active profile, you will see a link for AF469 under the profile column from your IMR homepage. 

For Air Force members with Self-Certify profiles, please visit the My IMR website for instructions on how to certify your condition and follow up actions. 

National Guard and Reserve Components 
If you’re in the National Guard or Reserve and your primary Military Treatment Facility is Offutt AFB, our BOMC assists with coordinating your profiles when you are seen off-base. If you live outside the Offutt AFB area and you have a civilian health care provider or PCM that indicates you need to be put on a profile please send a message to Offutt BOMC (search "Offutt BOMC") via the MHS GENESIS Patient Portal. You must send us medical records from your off-base provider through a MHS GENESIS message or fax them to 402-294-7314 to generate a profile from a military health care provider. Failure to do so will result in a cancellation of your request, without supporting documentation BOMC will not start any notifications to have your records reviewed for profiling purposes.
