Air Force 

Step 1 

Service embers will initiate the Medical Clearance Review by logging onto the My Individual Medical Readiness site. Air Force members will request a Medical Clearance Review for the following actions: 

  • Command Screening Board 

  • Commissioning 

  • Developmental Special Duty 

  • Initial Flying Class physicals 

  • Overseas PCS 

  • Palace Chase/Palace Front 

  • Professional Military Education attendance requiring an AF Form 422 

  • Retraining/reclassification 

  • Only CONUS PCS that require an AF Form 422 

 

Step 2 

All Clearances: Members must use the MHS GENESIS Patient Portal and send a message to Offutt Base Operational Medicine Clinic (search "Offutt BOMC") and follow instructions below for the type of clearance requested. Subject line will be one of the following (Overseas, Retraining, PME, PCPF, CSB), CONUS (if applicable), commissioning, Officer Training School. 

Overseas PCS or CONUS PCS if PPC requires an AF Form 422: Service members must send Medical Clearance Memorandums and your Report of Individual Personnel documentation. Members will ensure all IMR requirements (PHA, immunizations, and HIV tests) are current and will remain current 60 days past their report no later than date. Once the service member has initiated the process through the MyIMR site and sent a MHS GENESIS Patient Portal message to BOMC with appropriate paperwork, a comprehensive record review will ensure member is medically qualified for the PCS. MSME staff will contact member with instructions to schedule and complete specific medical tasks if indicated (i.e. face-to-face with PCM, PCS waiver, etc.). 

 
Virtual Out-processing (vOP) for CONUS & Overseas: Members must ensure all IMR requirements (PHA, immunizations, and HIV tests) are current and will remain current 60 days past their report no later than date . MSME will only sign off on your checklist if you are green and projected to stay green for 60 days past RNLTD. If this has not been completed by two duty days before final out-processing appointment, use the MHS GENESIS Patient Portal and send a message to Offutt BOMC to request MSME signoff. 

 

Other actions listed above (retraining, special duty, etc.): Members must include their CC/FM approval letter, AFPC email, AMS listing and/or any verbiage required on the AF Form 422. Failure to attach required approvals or supporting documentation will delay your request. You will be notified if you fail to send the required documentation through the MHS GENESIS Patient Portal. Continued failure to produce documentation will lead to your request being archived. 

 

OVERSEAS PCS: Send BOMC your Medical Clearance Memo and RIP via the MHS GENESIS Patient Portal. Family members will be cleared through EFMP at 402-294-6869. If you are PCSing to any base in Korea, go to the Immunizations Clinic and pick up a smallpox questionnaire. You will only be able to receive immunizations once you have been qualified by BOMC to PCS. Once your clearance is complete, your AF422 will be available on your MyIMR in the ‘Other’ column on the main page. Your Medical Clearance Memo will be signed and sent back to you utilizing MHS GENESIS Patient Portal.  
 
**CONUS PCS IF PPC requires an AF Form 422**: Submit OS PCS Medical Clearance request (there is no request option for CONUS in ASIMS). Once your clearance is complete, your AF422 will be available on your MyIMR in the ‘Other’ column on the main page. Your Medical Clearance Memo will be signed and sent back to you utilizing MHS GENESIS Patient Portal.   

 
RETRAINING/COMMISSIONING: Those selected for DSD, Special Duty, commissioning, or retraining with approved AFSCs from AFPC, and current AFSC DQs (9A). Send MyFSS eligibility email/commander MFR of intent to commission, via MHS GENESIS Patient Portal to Offutt BOMC.  
 
INITIAL FLYING CLASS PHYSICALS: IFCs are completed in the order received. The overall process (if a flying waiver is not required) can take months to complete as this exam is processed outside the 55th Medical Group.  

ENLISTED: Select the Retraining/IFC option. You must send one of the following to initiate clearance: MyFSS eligibility email, PPCs, or MFR of Commander Requirement for IFC. Once the IFC exam is approved by AETC, the 422 will be generated.  

OFFICERS applying for rated boards (initial or upgrade) will start the request by selecting the Retraining/IFC option; your records will be reviewed to start the IFC process. You must send one of the following to initiate clearance: PPCs or MFR of Commander Requirement for IFC 
 

PME (i.e., selected to attend ALS, NCOA or SNCOA): This is ONLY for those with a current AF Form 469 and who require an AF Form 422 to attend EPME. 
 
PALACE CHASE/PALACE FRONT: Please submit your AF Form 422 request ONLY when you are within 6 months of your final out date. SHPE must be complete PRIOR to issue of AF Form 422. Send the Recruiter Memorandum and/or letter. Inform BOMC of SHPE date via the MHS GENESIS Patient Portal for tracking purposes. ***IF YOU ARE COMPLETING SKILLBRIDGE***, and you cannot complete requirements prior to final out you must still complete SHPE before final separation. You may complete your SHPE at the nearest Veterans Affairs hospital if you are going to be out of state for Skillbridge. If this applies to you, please use the MHS GENESIS Patient Portal and message Offutt BOMC, and we can supply you your non-compliance MFR for your final out, you will still be required to finish while on Skillbridge. If you don’t complete SHPE it may/will impact your ability to process claims with the VA. 

 

Sister Service Medical Clearances 

Members use the MHS GENESIS Patient Portal and send a message to Offutt BOMC (search "Offutt BOMC") and follow instructions below for the type of clearance requested. Subject line will be one of the following (Navy Sea/Operational Duty Suitability Screening, Navy Shore Duty Suitability Screening, Sister Service Commissioning Physicals, etc.) 

 

Navy Sea/Operational Duty Suitability Screening: Service members must indicate the service and type of screening requested by sending a message to BOMC. BOMC will then send you a packet that needs to be filled and returned. Without return of the following, your Clearance will not be processed: 

  • Sea Duty orders 

  • Page 13 Memo 

  • NAVMED 1300/1 with demographics section completed 

  • NAVMED 1300/2 with demographics completed 

  • DD 2807-1, lines 1-29 completed 

PCM appointment may be needed if identified by BOMC, do not schedule to complete medical clearance unless contacted by BOMC to do so. 

  

Navy Shore Duty Suitability Screening: Members must attach their Page 13 Memorandum and indicate the service and type of screening requested. BOMC will process and return package to you via MHS GENESIS Patient Portal

  

Sister Service Commissioning Physicals: Please message BOMC via the MHS GENESIS Patient Portal for the steps to complete so that MSME may guide you through the process. You will require an in-person appointment with follow up requirements for testing (EKG, labs, chest x-ray, etc.), all requirements will be communicated to you from BOMC as you move through the process. 

 

Special Duty/Special Operations/Special Warfare Physicals: Please message BOMC via the MHS GENESIS Patient Portal for the steps to complete so that MSME may guide you through the process. There are many requirements that differ from special physicals.
