Primary Care Behavioral Health

Your provider or team nurse may refer you to our Integrated Behavioral Health Clinic team for more specific care coordination, or patients can self-refer. We have specialists on staff for questions, concerns, and short-term consultations regarding:

  • Depression, anxiety, grief
  • Smoking/tobacco cessation
  • Weight management
  • Marital/relationship concerns
  • Poor sleep quality (not for snoring, sleep study, or sleep apnea)
  • Pain management

IBHC providers do not prescribe or refill medications; however, our Behavioral Health care Facilitator registered nurse can assist with medication follow-up questions and care coordination with medications for depression, anxiety, or PTSD. Additionally, our BHCF nurse provides coaching interventions for patients newly-diagnosed with chronic diseases.
