Primary Care Behavioral Health

Your provider or team nurse may refer you to our Primary Care Behavioral Health (PCBH) team for more specific care coordination or patients can self-refer to PCBH. We have a specialists on staff for questions, concerns and short-term consultations regarding:

  • Depression, Anxiety, Grief
  • Smoking Cessation
  • Weight Management
  • Marital/relationship concerns
  • Poor sleep quality (not snoring, sleep study or sleep apnea)
  • Pain management
  • Tobacco Cessation

PCBH providers do not prescribe or refill medications, however our Behavioral Healthcare Facilitator Registered Nurse can assist with medication follow-up questions and care coordination with medications for depression, anxiety or PTSD. Additionally, our BHCF nurse provides coaching interventions for patients newly diagnosed with chronic diseases.

  • Closed on all federal holidays
  • Closed the third Thursday of every month, 11 a.m. - 4:30 p.m., for required staff training
