Disease Management

Disease Management is a team of specialized trained registered nurses available to help you achieve optimal health, independent of your chronic disease(s). Disease Management aims to help you improve your quality of life by preventing or decreasing risks for chronic disease complications.

Who we serve

Patients empaneled to the MTF with the following chronic health conditions:
  • Hypertension (High Blood Pressure)
  • High Cholesterol
  • Diabetes
  • Asthma
  • COPD
  • Congestive Heart Failure

What we provide

Once you meet with a disease management nurse, you will be in regular contact with our team. We will engage in regular one-on-one discussion about your health conditions, lab tests, current medications, prevention measures, health goals and any other identified needs. We collaborate with your PCM team and other specialty care teams, with a focus on helping improve your health and preventing further complications from the chronic disease process.
